How to Get a Disability Certificate in India: A Step-by-Step Process

You are standing outside a district hospital with a folder of reports, an old prescription, and a question nobody seems able to answer plainly: how do you actually get a UDID certificate in India without losing months to confusion?

The process is not mysterious. It only feels that way because the guidance sits scattered across government portals, hospital boards, and well-meaning advice that skips the details that matter. Here is the whole route, in the order you will walk it.

What the Certificate Actually Does

The Unique Disability ID, issued under the Department of Empowerment of Persons with Disabilities, is a single national document that proves eligibility anywhere in India. It replaces the folder of papers people carry from office to office.

Step 1: Check Your Eligibility

Assessment must place you at 40 per cent or above, and your condition must fall within the twenty-one categories recognised under the RPWD Act 2016. These span physical, sensory, intellectual, neurological and blood-related conditions, including blindness and low vision, hearing impairment, cerebral palsy, autism spectrum disorder, muscular dystrophy, thalassaemia, sickle cell disease, specific learning differences, and acid attack survivors, among others.

That 40 per cent figure is not a judgement of anyone’s worth. It is an administrative threshold, nothing more. If your assessment falls below it, support still exists, and plenty of it does not require certification at all.

Step 2: Gather Your Documents First

Most delays happen because people begin the application without everything ready. Before you start, collect:

  • Aadhaar card, for identity and e-KYC
  • Proof of address
  • A recent passport-size photograph
  • Medical records from a registered practitioner, including diagnosis and relevant test results
  • Any existing certificate, if you are renewing

Scan each one clearly. The application is online, so legibility matters more than photocopies do.

Step 3: Register on the Official Portal

Everything now runs through one place. Visit swavlambancard.gov.in, the official portal run by the Department of Empowerment of Persons with Disabilities, and select “Apply for UDID Card”.

You will register using your Aadhaar number or Aadhaar enrolment number, with verification by e-KYC. This single registration is the whole reason obtaining a UDID certificate in India is faster than it used to be. One application now covers both the certificate and the card.

Step 4: Complete the Application

Fill in your personal, employment, and condition details, then upload the documents from Step 2.

Check every field before submitting. A mismatched name or an unreadable upload is the most common reason an application stalls, and correcting it afterwards costs more time than checking it now. Note your enrolment number when it appears. You will need it to track everything that follows.

Step 5: Attend the Medical Assessment

Your application is routed automatically to the Chief Medical Officer or the Medical Board notified by your State Government for your district.

This is worth understanding clearly, because a great deal of advice online gets it wrong. You do not choose your medical authority, and there is nothing to shop around for. The competent authority is designated by the state, and your application goes to the right one on its own.

Carry your original documents to the appointment. The board will assess your condition and record both the type and the percentage.

Step 6: Track, Download and Use It

Return to the portal and use “Track Application Status” with your enrolment number and date of birth. Once approved, both your certificate and your UDID card become available to download and print directly from your account.
